Essential Membership – Basic Family Practice

$65 per month

Modern Edge Essential is your simple, flexible family practice membership.
You keep your insurance, and we give you a smaller, more personal practice with better access.

What’s Included

All the basics you expect from a good primary care clinic:
  • Annual physical/wellness exam
  • Acute/sick visits (same-week access)
  • Chronic condition management (blood pressure, diabetes, thyroid, etc.)
  • Medication refills & lab review
  • Referrals to specialists
  • Care coordination with hospitals and specialists
  • Routine lab ordering
  • Secure patient portal messaging during business hours
The membership fee gives you access to the practice and enhanced communication. Medical services are billed to your insurance plan when applicable.

How Billing Works

If You Use Insurance

For most visits and services, we will:

  • Bill your insurance plan for covered services
  • Collect any required co-pay, deductible, or coinsurance at the time of service

Your $65/month membership is separate from what your insurance pays and helps us keep a
smaller panel and more time with each patient.

If You Prefer Not to Use Insurance (Cash Visits)

Some patients prefer not to use their insurance, or they have a high deductible.


For members who choose to pay cash instead of using insurance:

  • Member Cash Visit Rate: $100 per visit
  • (This applies to standard office visits when not billed to insurance.)

Cash-Pay Discounted Labs

Members may choose either:

  • To bill labs through their insurance, or
  • To use discounted cash-pay lab pricing through our partnered lab (Quest Diagnostics).

If you choose the cash option:

  • We’ll give you an estimate for your labs before they’re drawn
  • You pay the lab’s self-pay rate, which is often much lower than hospital-based lab pricing
  • Great option for patients with high deductibles or no insurance
  • Exact lab prices vary by test and lab. We’re happy to provide a quote before ordering your labs.
